Periodontal Disease
DESCRIPTION
Periodontal disease refers to disorders of the gum and bone that hold the teeth.
Gingivitis is a general term for inflammation of the gum and is the early warning
sign of potential problems. If untreated, it often leads to periodontitis, the major
cause of tooth loss after the age of 35. This more chronic condition involves an
infection of the periodontium (the gingiva, ligament, and bone structure holding
the teeth). If it continues unchecked, the infection will spread to the bone in
which the teeth are rooted. The bone then resorbs and the teeth are slowly detached
from their supporting tissues. Ischemia (poor tissue perfusion of oxygenated blood)
is yet another condition that may also negatively affect the health of the jaw bone.
When ischemia occurs in bone, the result is osteonecrosis or death of the bone.
SIGNS/SYMPTONS
Inflammation of the gums with bleeding is one of the primary symptoms of gingivitis.
Other symptoms include swelling, redness, pain, and difficulty in chewing. Once
the condition has progressed to periodontitis, the symptoms are more severe, more
chronic, and lead to the loss of teeth. In the case of ischemia resulting in osteonecrosis
of the jaw bone, there is severe pain and possible loss of teeth.
CAUSE
The conditions of gingivitis and periodontitis are primarily caused by residual
food, bacteria, and calcium deposits (tartar) that collect in the spaces between
the gum and lower part of the tooth crown. Contributing factors are poor diet, poor
dental hygiene, and general poor health with a weakened immune function. Osteonecrosis
is characterized by poor blood flow into bone marrow structures. This is often due
to reduced fibrinolytic and thrombolytic activities that leads to the formation
of blood clots. As a result, there is reduced blood flow,reduced nutrient and oxygen
supply to the bone tissues, and ultimately necrosis.

Hydrolytic enzymes (proteases and amylases) help break down food debris and enhance
their removal and flushing by the saliva. Furthermore, enzymes help prevent and
dissociate some of the periodontal bacteria attachments (plaque and tartar buildup).
Additionally, the enzymes help heal sores and cuts in the mouth that otherwise may
serve as gates for bacteria into the body to cause septicemia. The oral administration
of proteolytic enzymes may remedy the blood clotting and improve blood flow to the
necrotic bone tissues, revitalizing them and preventing further tissue destruction
and infection. Protease controls the oversecretion of cytokines, which increases
immune modulation. Protease has also demonstred increased tumorcidal activity of
polymorphonuclear neutrophils (i.e., the defense mechanism of our white blood cells).
